in order to understand what to do and when, and what not to do and when, 00:00:54.000 |
and to come up with strategies that are very adaptive for us to move forward in the context of relationships, 00:00:59.000 |
work, school, and athletics, and on and on, there's really no separation between executive function and memory. 00:01:06.000 |

- Yeah, you may not remember me, but I remember you when I was a first year graduate student and you showed up at Berkeley, 00:06:36.000 |
one of the first people to really bring functional imaging of the human brain to Berkeley, 00:06:40.000 |
bring a neurology and a clinical emphasis to the neuroscience studies there, and it's really just blossomed. 00:06:48.000 |

Maybe we'll just start off with a few of the basics and do a little functional neuroanatomy lesson for folks. 00:07:07.000 |
Not to scare anyone, don't worry, this will be accessible to everyone. 00:07:10.000 |
And just talk about the frontal lobes and prefrontal cortex and a little bit of what those structures do. 00:07:16.000 |
Because many times on this podcast, I've said, 00:07:18.000 |
okay, the neural real estate right behind your forehead is involved in context and planning, et cetera, 00:07:25.000 |
How should we think about what the frontal lobes do and their various roles in health and disease? 00:07:34.000 |
There's the frontal lobes, parietal, temporal, occipital, 00:07:36.000 |
and the frontal lobes probably do take up more territory than the other lobes, 00:07:43.000 |
And within the frontal lobes, I'm going to use the frontal lobes probably in our conversation a lot, 00:07:49.000 |
but what I really mean is the prefrontal cortex. 00:07:51.000 |
So within the frontal lobes, there's also areas that are important for motor function as well. 00:07:57.000 |
But when we're talking about the frontal lobes 00:07:59.000 |
and talking about its involvement in higher-level cognitive abilities, 00:08:04.000 |
and this is what's considered sort of the highest level of cortex in the brain. 00:08:09.000 |
So yeah, when you think about it, people assign it all sorts of functions. 00:08:14.000 |
Almost every function you think of, people sort of put into the frontal lobes. 00:08:17.000 |
But I think what we've all kind of moved towards is this idea of executive function, 00:08:23.000 |
this ability to plan, to organize, to really transfer our thoughts into an action 00:08:31.000 |
and really to be guided by goals and intentions 00:08:34.000 |
and not be kind of ruled by sort of just automatic behaviors. 00:08:39.000 |
A word we use in neuroscience is called cognitive control. 00:08:43.000 |
Cognitive control, executive function is what we attribute to the frontal lobe. 00:08:47.000 |
And so you can think of it as the CEO of the brain or the conductor of the orchestra, 00:08:54.000 |
really the part of the brain that's really controlling the rest of the brain. 00:08:59.000 |
So yeah, if you had to choose which part you wanted to not leave home, it's your frontal lobes. 00:09:06.000 |
Speaking of which, what are some of the symptoms of mild frontal lobe damage 00:09:15.000 |
damage brought about either through neurodegenerative disease or physical injury? 00:09:20.000 |
I know we're going to talk a bit about both today, or a lot about both. 00:09:24.000 |
But how would lack of executive function show up, maybe on kind of a subtle level? 00:09:32.000 |
Yeah, I mean, first I should say is that it shows up all the time because when-- 00:09:37.000 |
and frontal lobe behavior is probably much more prevalent than we realize. 00:09:42.000 |
Certainly we think about it when you have a brain injury to the frontal lobes. 00:09:46.000 |
And there's lots of neurological disorders like stroke and traumatic brain injury 00:09:49.000 |
and Alzheimer's disease that can affect the frontal lobe. 00:09:52.000 |
And there's a number of psychiatric disorders, obsessive-compulsive disorder 00:09:56.000 |
and schizophrenia and depression that are thought to be frontal lobe dysfunction. 00:10:00.000 |
But when you're sleep-deprived and when you're stressed and just normal aging, 00:10:04.000 |
the frontal lobe seems to be the first system that's affected 00:10:08.000 |
because it really is involved in the highest level. 00:10:10.000 |
So when we're having a bad day, when we're having difficulty sort of setting priorities, 00:10:15.000 |
when we're having difficulties achieving the goal that we've set out, 00:10:19.000 |
when we get distracted, when we're not able to sort of adapt and be flexible, 00:10:25.000 |
these are all the type of things that reflect that our frontal lobes are not functioning. 00:10:32.000 |
Approximately what age does the frontal lobe circuitry come online, so to speak? 00:10:38.000 |
I mean, when I see a baby, babies can orient their eyes towards things, 00:10:43.000 |
but they're rather reflexive in where they'll place their eyes. 00:10:46.000 |
But by the time kids are three or four, they can certainly play with blocks 00:10:52.000 |
or interact with other children or their parents. 00:10:55.000 |
But it seems that full functionality of the frontal lobes is really gradual. 00:11:00.000 |
At least that's my non-clinically trained assessment. 00:11:04.000 |
Yeah, I mean, it's a really tough question to know when they're fully developed 00:11:09.000 |
When MRI was introduced and we were able to sort of image the brain in a non-invasive way, 00:11:14.000 |
then studies did start to come out trying to sort of map out at what age 00:11:18.000 |
does your frontal lobe fully develop, and it seemed like it was early into your 20s. 00:11:24.000 |
You know, I always say that it's not surprising that you can't rent a car until you're 25. 00:11:31.000 |
Nurse scientists did as to when your frontal lobes have, you know, 00:11:34.000 |
when your decision-making skills are at their highest. 00:11:38.000 |
Until your 20s is probably before your frontal lobes are fully developed. 00:11:42.000 |
And it's a really interesting question, is why does it take so long? 00:11:46.000 |
It's the area of the brain that takes the longest to develop, and why is that? 00:11:51.000 |
I think there's a reason. I think that this sort of slow development of frontal lobes 00:11:56.000 |
allows us to explore, allows us to think about novel ways of solving problems, 00:12:04.000 |
If they were shut off earlier, it would lead to maybe a much more sort of rigid, 00:12:10.000 |
kind of, you know, less flexible kind of behavior that we'd see in things. 00:12:16.000 |
So I think that it helps to take a long time to develop, 00:12:20.000 |
but also it obviously leads to some problems sometimes in adolescence, 00:12:30.000 |
in just the sheer number of physical movements that a child makes? 00:12:34.000 |
So, for instance, in a classroom of, you know, let's say, you know, fourth graders, 00:12:41.000 |
oftentimes there'll be a range of apparent ability of kids to sit still or to listen. 00:12:49.000 |
Do we think that the kid that's having a hard time focusing and listening 00:12:53.000 |
to instructions or steadying their body when they're told to sit still-- 00:12:57.000 |
I don't know if they still tell kids to sit still, 00:12:59.000 |
but they were telling me to sit still when I was a kid. 00:13:02.000 |
Is that somehow reflective of a, you know, slightly lagging frontal lobe function 00:13:08.000 |
and maturity, whereas the kids that can sit still and stoic and focus, 00:13:14.000 |
does that mean that they're a little bit more accelerated along that trajectory? 00:13:19.000 |
I mean, the frontal lobe is a big territory, and we can get into it, 00:13:22.000 |
but there's, you know, the frontal lobe probably has 25 different subregions within it, 00:13:28.000 |
and so grossly we think about the frontal lobes as the lateral portion of the frontal lobes, 00:13:33.000 |
which is involved in these executive function, 00:13:36.000 |
probably supports these executive function abilities, 00:13:38.000 |
but then we've got another part of the frontal lobes called the orbital frontal cortex, 00:13:41.000 |
which is probably involved more in social and emotional behavior. 00:13:45.000 |
So, you know, again, when we think about frontal lobe behaviors, 00:13:49.000 |
you have to break--there's so many different types of frontal lobe behaviors. 00:13:52.000 |
So that type of behavior, which may be involved in sort of being able to inhibit, you know, 00:13:57.000 |
your motor movements or maybe not being distracted, 00:14:00.000 |
may reflect that that system is a little bit delayed, 00:14:02.000 |
but it could be that another system, the one that's involved in planning and organized, 00:14:06.000 |
is more developed, and I do think they develop at different trajectories. 00:14:13.000 |
So with the frontal lobes essentially serving an executive or CEO-type function, 00:14:19.000 |
goal-directed behavior, intentions, cognitive control, these are the terms you used. 00:14:26.000 |
Where are the rules? What do the rules look like? 00:14:30.000 |
You know, when I think about brain function, which I've spent a lot of my life thinking about, 00:14:34.000 |
I think about chemical and electrical signaling between neurons, 00:14:39.000 |
different neurons communicating more or less at a given moment, 00:14:43.000 |
reflecting some sort of circuit, as we call it, and then some behavior or some decision comes out. 00:14:49.000 |
And if I, for instance--I have to get my driver's license renewed soon. 00:14:53.000 |
So if I go to the Department of Motor Vehicles, what a lovely experience that is. 00:14:57.000 |
The moment I get there, I sort of lock into a certain rule set. 00:15:05.000 |
When I'm with my friends versus when I'm with my parents, different rule sets. 00:15:09.000 |
And it seems that the frontal lobe is really good at drawing on context based on knowledge of where one is, 00:15:17.000 |
and then coming up with kind of algorithms that are appropriate or inappropriate to run in that context. 00:15:25.000 |
Are they of the, "Okay, shut down all cursing in this environment. 00:15:34.000 |
I mean, when it really comes down to it, it has some interesting philosophical aspects, too, 00:15:38.000 |
because just be yourself, be authentic, be vulnerable. 00:15:42.000 |
You know, all these things make sense, but, of course, one needs to be appropriate with the context. 00:15:55.000 |
Right, because that's a pretty common example of our patients, that they don't follow the rules. 00:16:00.000 |
You know, if you're sitting in the doctor's office and the phone rings, you know not to pick up his phone, 00:16:07.000 |
but the patients don't, and they may pick up the phone. 00:16:11.000 |
There's this Dr. Lamite, who's a neurologist from France, published these beautiful papers in the '80s 00:16:16.000 |
of all these things that patients did that broke the rules 00:16:20.000 |
and just kind of pulled to their environment without having any context to it. 00:16:25.000 |
If he put a pair of glasses on the table and didn't ask them to put them on, 00:16:29.000 |
they would put them on even if they had a pair of glasses on already, 00:16:32.000 |
or he took them to their apartment and they saw the bed, and they'd jump into the bed and go under the covers. 00:16:38.000 |
He had a nurse, and he put a blood pressure cuff there, and she picked up the blood pressure cuff 00:16:43.000 |
and just started taking his blood pressure, again, not asking him to do any of these things. 00:16:47.000 |
And so they just don't follow sort of the social rules, but they're there. 00:16:55.000 |
If you ask these patients, "Was that the appropriate thing to do?" they'll say, "No." 00:17:01.000 |
Yeah, they say, "No, I'm not supposed to answer your phone." 00:17:05.000 |
Oh, wow, so they know better, but they can't control the impulse. 00:17:08.000 |
Exactly. So it's not a breakdown that the rules disappear. 00:17:12.000 |
It's that they can't apply the rules properly. 00:17:17.000 |
And that's true for a lot of patients, even with kids. 00:17:20.000 |
You know, you tell them, "Don't have anything to eat before dinner because we're having dinner," 00:17:25.000 |
and then they're sitting there having a sandwich, and you say, "What did I just tell you?" 00:17:28.000 |
You say, "Well, don't eat, but I'm hungry," right? 00:17:31.000 |
Another sort of example is sort of the frontal lobe's not completely kind of developed. 00:17:35.000 |
So when I think about rules, I think about the brain. 00:17:39.000 |
You know, the brain processes information, obviously, but it also stores information. 00:17:43.000 |
The most important thing it does is store all sorts of information all over the brain. 00:17:48.000 |
And I think what the frontal lobes do is they store rules. 00:17:52.000 |
And what's interesting about the way it stores rules is they seem to store the rules in a hierarchical fashion. 00:18:00.000 |
And what I mean by that is that there's different levels to rules. 00:18:08.000 |
I tell the story a lot about my good friend Bob Knight when he hits a ball into the woods, 00:18:13.000 |
and he has to try and hit the ball out of the woods. 00:18:16.000 |
He's holding on to all different levels of rules on how to successfully get his ball back towards the green. 00:18:22.000 |
So the most simplest one is just like where is the, you know, I've got to maintain the orientation to get to the flag, you know. 00:18:34.000 |
He knows that if he kicks the ball, it's a penalty, so he's not going to do that, right? 00:18:38.000 |
And then another higher level rule might be if I just keep doing this, you know, this is going to be healthy for me. 00:18:45.000 |
So he's storing all this information at sort of different levels of hierarchy, 00:18:49.000 |
and he's applying it to ultimately achieve this very simple act of, or not so simple act of hitting the off ball. 00:18:56.000 |
So yeah, so I think about sort of the frontal cortex is able to call upon the rule in the appropriate context. 00:19:04.000 |
And if you don't have your frontal lobes, it doesn't get pulled up properly. 00:19:11.000 |
There's no way I can imagine that one can be born into the world with these rule sets intact. 00:19:18.000 |
I think about the two marshmallow experiment that's sort of famous now, 00:19:23.000 |
where kids are offered to eat one marshmallow right away or defer and get two marshmallows. 00:19:30.000 |
These adorable videos of the kids and the various strategies they use, like turning away, poking the marshmallow. 00:19:36.000 |
And, you know, there's some debate ongoing as to whether or not success or lack of success 00:19:42.000 |
in deferring to the two marshmallow reward is predictive of other things in life. 00:19:48.000 |
But leaving that aside, am I correct in assuming that that task is a frontal lobe task? 00:19:56.000 |
You can have one marshmallow now or wait patiently 00:20:00.000 |
and then with an overcome the craving for that one marshmallow, and then you'll get two. 00:20:05.000 |
And presumably that experiment is engaging the frontal lobes. 00:20:12.000 |
And, you know, we can only speculate, but some kids are able to defer, some are not. 00:20:19.000 |
And I can imagine that at that age there's a lot of neuroplasticity, 00:20:24.000 |
strengthening and weakening of connections in the brain in an experience-dependent way. 00:20:29.000 |
So does that mean that children and perhaps adults as well can train up their prefrontal cortical abilities 00:20:38.000 |
to strategize and defer in a way that's adaptive? 00:20:41.000 |
Absolutely. I mean, definitely you can learn strategies to not only sort of learn rules, but how to apply goals. 00:20:49.000 |
When you start to think about that task in particular, some of it has to do with sort of maintaining a goal 00:20:55.000 |
and maintaining a goal at different, you know, time scales, right? 00:20:59.000 |
And children tend to sort of act on goals that are much more short on a shorter time scale. 00:21:04.000 |
You know, I'm going to have the sandwich right now because I'm hungry, as opposed to wait until dinner, 00:21:10.000 |
And so, yeah, this default is sort of the shorter--you can learn that maintaining a longer-type goal 00:21:17.000 |
can be much more beneficial than the short-term goal, even though it doesn't seem obvious. 00:21:22.000 |
And we all learn that, right? As we get older, we keep our eye on the ball of sort of more long-term goals, 00:21:29.000 |
and that's very predictive of how successful we can be. 00:21:34.000 |
And that's what the prefrontal cortex does. It maintains goals and then applies those goals. 00:21:39.000 |
And if you don't apply them, then all of this executive function breaks down. 00:21:45.000 |
Do you think that these algorithms and rules that the prefrontal cortical circuitry can learn 00:21:54.000 |
So for instance, my first year of college was a disaster for reasons that aren't interesting right now. 00:22:01.000 |
But then when I came back my sophomore year, really spring of my freshman year, I was like, 00:22:07.000 |
And so one of the things I used to do was I would study and I would set a timer so I refused to get up. 00:22:13.000 |
Even if I had to use the restroom very, very badly, I would set up all sorts of behavioral constraints. 00:22:20.000 |
And I like to think that I was building up my prefrontal ability to refocus on the material. 00:22:25.000 |
And fortunately for me, there were no smartphones back then. It was much easier, the internet. 00:22:29.000 |
Now we had email, but no real internet browsing to speak of. 00:22:33.000 |

and put together all sorts of rules, and they're really good at that task very quickly. 00:24:07.000 |

But I think we've learned on how to try and make it translate to real life. 00:24:16.000 |

where they've been very successful in teaching patients how to improve your executive function 00:24:34.000 |
and how to make that translate into your real world. 00:24:37.000 |
But it's very hard work. It's very therapist-driven. 00:24:45.000 |
For example, they develop individual projects like planning a meal or planning a family vacation 00:24:51.000 |
or planning a podcast, and then they work through what's involved in that sort of very specific project, 00:24:57.000 |
how you stay focused, how you don't just get distracted, how you keep your eye on the ball, 00:25:02.000 |
how you break it down to sub-goals, how you monitor what you're doing, 00:25:08.000 |
how you don't let anxiety and procrastination get involved. 00:25:14.000 |
But when you add all that to it in a very disciplined way over the course of many hours, many weeks, 00:25:21.000 |
Patients and individuals just say, yeah, I'm just better at doing things. 00:25:25.000 |
I mean, the whole goal is to do things, right, and I'm just better at it. 00:25:28.000 |
I don't know what it is, but I'm not just better at what you taught me. 00:25:33.000 |
So I do have a lot of hope that these kind of therapies will generalize to people's real life. 00:25:40.000 |

the connections from the frontal cortex to, let's call it, circuitry that controls reflexive behaviors. 00:26:23.000 |
What is the nature of that circuitry, and can we make any general statements? 00:26:28.000 |
Like, does the frontal cortex really serve to provide a quieting, suppressive function on reflexes, 00:26:36.000 |

And then, of course, we could talk about how it's connected to the body as well, 00:27:26.000 |
how it controls heart rate and respirations as well, so it's not just the brain. 00:27:30.000 |
But it's really interesting, like you said, is it really just sort of maintaining, 00:27:35.000 |
telling you what's relevant and what's not relevant, or is it allowing you to switch? 00:27:41.000 |
It definitely, what we call, sends these top-down signals. 00:27:44.000 |
It's sending signals to the other brain about what you should be paying attention to 00:27:48.000 |
and what you shouldn't be paying attention to. 00:27:50.000 |
So, for example, we've done studies with functional imaging 00:27:54.000 |
where we have them look at pictures of faces and scenes, 00:28:00.000 |
Your visual cortex has areas that can process faces and process scenes. 00:28:05.000 |
But sometimes we have you just want to pay attention to the faces and not the scenes, 00:28:10.000 |
and other times we want you to pay attention to the scenes and not the faces. 00:28:13.000 |
Well, you know, even though it's getting the same bottom-up visual input, 00:28:18.000 |
the prefrontal cortex will show greater activity to the relevant information. 00:28:24.000 |
It's sending a signal saying, "Pay attention to the faces, ignore the scenes," or vice versa. 00:28:30.000 |
So it's directing all of this information that we've been rewarded with to what's relevant. 00:28:35.000 |
But at the same time, it's also allowing us to switch. 00:28:39.000 |
If we now have to go switch to another task, it says, "Okay, this is not important now. 00:28:44.000 |
We're going to move over to this other task." 00:28:47.000 |
So there's many different components of how it can kind of control behavior, 00:28:51.000 |
but it does all of these things in this incredible way that we still don't completely understand, 00:28:56.000 |
but we know that the source of all of this control is coming from the prefrontal cortex. 00:29:02.000 |

So much of things like stoic philosophy and even online wellness culture 00:34:54.000 |
are about having routines, overcoming reflex by just having recipes, scripts to follow each day. 00:35:04.000 |
I certainly try to have my mornings be as what I call linear as possible. 00:35:10.000 |
And I find it's much easier in the earlier part of the day to just decide, 00:35:14.000 |
here's what I'm going to do, write out a list, do things in a certain sequence. 00:35:17.000 |
If I don't do that, I go non-linear as I refer to it, and we'll get distracted and things of that sort. 00:35:23.000 |
But earlier you mentioned sleep deprivation can impair frontal lobe function. 00:35:28.000 |
It does seem that as the day progresses, and certainly in the middle of the night, 00:35:33.000 |
it just becomes much harder to control our thinking, maybe even our behavior, and certainly our emotions. 00:35:42.000 |

and half of it is involved in these high-level executive functions, 00:35:57.000 |

We call it the paralimbic system that's involved in social and emotional behavior. 00:36:05.000 |
And so there's this intimate back and forth between these two areas of the cortex. 00:36:10.000 |
If you have just damage to these areas that are kind of in the frontal lobe, 00:36:15.000 |
you will have many different impairments that we would call sort of social or emotional impairments, 00:36:21.000 |

and then you'll have the opposite where patients with the lateral damage will have executive functions, 00:36:31.000 |
But in real life, when we have both of these intact, they're communicating with each other, 00:36:36.000 |

We make bad decisions in stressful situations or situations we're not comfortable with. 00:36:47.000 |
It's where we might make a better decision if it's a quiet place. 00:36:54.000 |

You can sort of get into a routine and learn how to do things if you have very much planned out, 00:37:03.000 |

I certainly use a smartphone from waking till sleep, 00:37:37.000 |
generally not in the middle of the night if I can avoid it, and I generally avoid it, 00:37:41.000 |
but I'm trying to take what we've discussed thus far 00:37:45.000 |
and superimpose the notion of smartphones and ask, "What are the rules? 00:37:53.000 |
What are the algorithms that we're learning when we use these devices?" 00:37:56.000 |
And I'm not calling them adaptive or maladaptive. 00:38:01.000 |
I'm sure it makes it easier for doctors to communicate on the ward and for clinic. 00:38:11.000 |
But contained in this small device, there are things like, for instance, text messaging, 00:38:17.000 |
where unlike 20 years ago, we can have four or five different conversations very quickly 00:38:24.000 |
There's a task switching element that was just not present in our life prior to that. 00:38:33.000 |
Social media in particular, this notion of being able to scroll. 00:38:37.000 |
So if we really step back from this, move one's thumb and access hundreds, if not thousands, 00:38:43.000 |
of video content, each of which has a distinct context. 00:38:49.000 |
And so I have to imagine that kids and adults have frontal cortices that are learning these rules. 00:38:56.000 |
And the rule is move your thumbs, stay engaged, emotions, either positive valence emotions 00:39:06.000 |

having kids is watching them navigate in their cars to places totally dependent on Google Maps. 00:43:26.000 |

Right, where you had to really figure out, you know, you had to go to a certain place 00:43:37.000 |

I can hold that in mind too until I put it down. 00:45:59.000 |
So when you think about it, it's a very important ability that we have that we do very flawlessly. 00:46:08.000 |
And what I've learned more about working memory is the working part of it. 00:46:12.000 |
It's not just this passive holding information in mind, but it's being able to do things with the information. 00:46:18.000 |
It's being able to--you know, when we do a math problem, which we don't do that much now that we have calculators, 00:46:24.000 |
but if you do that in your head, you're able to sort of manipulate the information and do the different parts of the problem. 00:46:30.000 |
Or even if you're trying to find someone in a crowd and you're holding onto some face, 00:46:35.000 |
you're able to hold that face in mind and cross-check it and search. 00:46:39.000 |
And so there's operations to working memory. It's not just this passive maintenance. 00:46:46.000 |
So when we started to think about working memory in that way, we started to realize how important it is for-- 00:46:51.000 |
I think of it as the foundation for cognition. Just think about reading comprehension. 00:46:58.000 |
If you can't hold in mind what's going on earlier in the conversation or when you're reading a book, 00:47:06.000 |
So it just predicts all these abilities that allows us to read, to plan, to organize, 00:47:14.000 |

this is information that gets consolidated and gets put into a more durable form that we call long-term memory. 00:48:43.000 |
And the interesting thing about memory is that these are separate systems. 00:48:47.000 |
Everything from working memory just doesn't pass into long-term memory. 00:48:50.000 |

I mean, when I realized that I could test these drugs in healthy individuals, 01:02:49.000 |
that if I gave them in low enough doses, they were safe, 01:02:53.000 |

And this has been a general problem with neurology. 01:03:30.000 |
It's always focused on how can we develop a treatment for Alzheimer's 01:03:36.000 |
as opposed to how can we develop a treatment for working memory dysfunction, 01:03:43.000 |
So the answer to your earlier question is these drugs are very safe. 01:03:47.000 |
We give them in such low doses to healthy individuals, 01:03:49.000 |
they can't even tell the difference between the placebo and the drug. 01:03:57.000 |
So they're not buzzing, thinking like, "Oh, this feels good, 01:04:02.000 |
They don't even know their working memory is better 01:04:04.000 |
until we show them that their working memory is better. 01:04:10.000 |
Bromocriptine is but one of the dopamine agonists. 01:04:13.000 |
You can think of a few other, cabrogoline, like other things like that. 01:04:19.000 |

giving neuromodulators acetylcholine, dopamine, norepinephrine, 01:05:37.000 |
and there was a lot of animal literature sort of supporting that this would work in humans. 01:05:43.000 |
But what was more striking to me was that it wasn't always just a single neurotransmitter. 01:05:50.000 |
There were studies where you'd give dopamine and it wouldn't do anything. 01:05:53.000 |
You'd give acetylcholine and it wouldn't do anything. 01:05:55.000 |

As far as I know, it's an epinephrine or norepinephrine agonist. 01:06:49.000 |

well, that can indirectly help your executive function. 01:07:47.000 |

is because of the sleep deprivation that it can cause. 01:27:57.000 |
Not only that, but one of the most common symptoms 01:28:00.000 |
that my patients with concussion have is their sleep is disruptive. 01:28:07.000 |
Almost every neurological disorder, my patients complain of their sleep. 01:28:10.000 |
And I started asking, not a lot of neurologists ask you how you sleep. 01:28:17.000 |
one of the first things my attending would do when we got to the ward 01:28:28.000 |
And we still don't understand why just brain injury does that. 01:28:32.000 |
So almost every concussion patient says, "I'm not sleeping well," 01:28:39.000 |
So optimizing sleep, obviously optimizing nutrition. 01:28:45.000 |
It used to be that we used to recommend, you know, you had a concussion, 01:28:48.000 |
you should don't go to work, you know, just take it easy for a while. 01:28:58.000 |
But now the idea is that you should really get up and moving. 01:29:04.000 |
You don't want to give yourself more of a headache or more light sensitivity. 01:29:08.000 |
But as much as you can tolerate is the thought these days 01:29:15.000 |
and then really getting your brain back working. 01:29:20.000 |
they're off from work for a couple of weeks and they feel fine 01:29:24.000 |
and then the first day of work is a complete disaster 01:29:26.000 |
because until you actually test it in real life, 01:29:29.000 |
you don't know what kind of troubles you have. 01:29:34.000 |
but I do recommend going back trying to build up these skills again. 01:29:39.000 |
And then I think we need to develop therapies that people will use. 01:29:45.000 |
You know, things like goal management training, 01:29:47.000 |
which involves a therapist, you know, health insurance doesn't pay for this. 01:29:51.000 |
So 99 percent of my patients don't get any help, you know, 01:29:59.000 |
But now we talked about technology, things like brain HQ. 01:30:07.000 |
So Mike Merzenich, which I know you've talked about with Eddie, 01:30:16.000 |

"I'll do brain training," you'll be overwhelmed with things 01:30:52.000 |
and you don't know what works and what doesn't work. 01:30:54.000 |
Yeah, I think the work that Merzenich and colleagues have done, 01:30:58.000 |

But I will say, I think Mike's work has been tremendous. 01:31:09.000 |
20 years ago, everyone was talking about neuroplasticity in critical periods. 01:31:14.000 |
They gave a Nobel Prize to it, to my scientific great-grandparents, 01:31:17.000 |
David Hubel and Torrance and Wiesel, and they deserved that Nobel Prize. 01:31:22.000 |

your primary care physician, every year from your yearly physical, 01:37:48.000 |
they examine every organ in your body except your brain, 01:37:53.000 |
your lungs, your heart, your skull system, your skin, 01:38:07.000 |

I'm involved in something called the Brain Health Project, 01:38:19.000 |
which is at UT Dallas, which is their goal as a study 01:38:27.000 |

But if we actually had a way to sort of track it 01:42:24.000 |

and lung disease and skin and things like that, 01:42:30.000 |

it's so many factors that are probably involved 01:50:04.000 |

and then I felt lousy for a couple hours after it wore off. 01:52:38.000 |

because it's also metabolized in the periphery 01:52:55.000 |
Sinemet came along which has this de-carboxylase inhibitor 01:52:59.000 |

tend to be something that families can help compensate for, 01:54:06.000 |

but it's really just a purely symptomatic therapy. 01:54:17.000 |
And the more socialization that patients get, 01:54:23.000 |
that if you showed patients family movies or family albums, 01:54:28.000 |
it was better than any drug you could give them 01:54:49.000 |

in the airline network, you've got all these hubs all over the country. 02:12:42.000 |
In the United States, for example, you've got Chicago as a hub 02:12:46.000 |
and there's other hubs, Milwaukee or Cincinnati, 02:12:49.000 |
but they have very different functions in the network as a whole, right? 02:12:54.000 |
If you're trying to get from New York to San Francisco, 02:13:00.000 |
even though you're not going through Chicago, 02:13:02.000 |
if Chicago is shut down, you're probably going to get delayed 02:13:06.000 |
because it just has this huge impact on the whole system. 02:13:09.000 |

So what's exciting to me is not only is that making us thinking about disease differently 02:13:46.000 |
because now we're starting to think about how are diseases affecting these hubs, 02:13:53.000 |
when you look at Alzheimer's disease and you look at schizophrenia 02:13:58.000 |
it's not just that there's some microscopic change in some neuron. 02:14:01.000 |
It seems to be affecting hubs in the brain that are affecting the whole network, 02:14:06.000 |
and so we have a different target to go after for treatments. 02:14:10.000 |
What can we do to sort of boost a hub that's been damaged 02:14:14.000 |
as opposed to thinking about it in a less specific way? 02:14:18.000 |
And then also, as we really start to learn about how the brain is organized in these networks, 02:14:25.000 |
we've also learned that measuring your network structure is very predictive of your well-being 02:14:35.000 |
So there's a metric called modularity, which measures how organized your brain networks are, 02:14:42.000 |
and the brain is made up of different modules, different networks, 02:14:47.000 |
and these networks can either be very communicating with each other 02:14:52.000 |
and the more segregated they are, we call that more modularity. 02:14:59.000 |
They're modular, and it turns out we can measure that with fMRI. 02:15:08.000 |
and then we can measure how modular your brain is versus my brain, 02:15:12.000 |
and all of us are very different levels of modularity. 02:15:15.000 |
Is it more advantageous to have more modularity as opposed to less? 02:15:19.000 |
Yeah, it turns out that it seems to be more advantageous to have more, 02:15:23.000 |
More separateness of brain function between areas. 02:15:26.000 |
Yeah, that the networks are sort of more independent. 02:15:28.000 |
That doesn't mean they don't talk to each other, but at sort of baseline, they're more independent. 02:15:35.000 |
Yeah, they're more independent as opposed to less independent with each other. 02:15:42.000 |
Joint development are what we call polyinnervated. 02:15:45.000 |
That's why babies can move their limbs, but not with a lot of coordination. 02:15:48.000 |
Look at a one-year-old trying to eat spaghetti, for instance. 02:15:54.000 |
There's a lot more precision and movement largely due to removal, more modularity of connections. 02:16:01.000 |
So we did a study where we took 12 traumatic brain injury patients and measured their modularity. 02:16:06.000 |
So you get a number, you just get a modularity index for each of the 12 people, 02:16:11.000 |
and then they underwent this goal management training, 02:16:14.000 |
and we were able to predict who was going to improve on the training. 02:16:19.000 |
Those who had started off more modular benefited more from the training. 02:16:24.000 |
And it's turned out that this has been a very robust finding across studies now, 02:16:28.000 |
across different training, different young, old patient populations. 02:16:34.000 |
It's also predicted things like whether someone in a coma is more likely to do well, 02:16:40.000 |
or if someone who's older is going to have a certain amount of cognitive decline, 02:16:44.000 |
or someone's going to respond to behavioral therapy if they're obsessive compulsive. 02:16:48.000 |
So there's something about this brain state that not only we can measure, 02:16:52.000 |
but actually is giving us insight into the interventions that we're doing, 02:16:57.000 |
which again is going to be much more helpful in determining what helps and what doesn't help 02:17:03.000 |
if we know sort of what the state is before we start the intervention. 02:17:08.000 |
So interesting and makes me think many things, but I'll just focus on two of them. 02:17:14.000 |
One is I love this idea that you and others are starting to look at brain network activity 02:17:20.000 |
as opposed to overemphasizing the role of, say, prefrontal cortex or hippocampus, 02:17:25.000 |
understanding more that those are hubs in a larger theme of activation, 02:17:31.000 |
because if I had one wish for science communication, 02:17:34.000 |
it's that people would, yes, learn some terms like dopamine and frontal lobes. 02:17:39.000 |
but to understand that if you really want to be able to work with the information 02:17:44.000 |
in a way that's beneficial, you need to think about verbs, not nouns. 02:17:50.000 |
and those action states are always involving multiple areas. 02:17:55.000 |
Just like you can't talk about running as just like quadricep and hamstring flexion 02:18:02.000 |
You can break it down that way, and it's useful to know that, 02:18:06.000 |
but ultimately you're talking about gait and stride 02:18:09.000 |
and things that have a real verb action to them, 02:18:13.000 |
and we haven't had so much of that for the nervous system at a circuit level. 02:18:17.000 |
We've been able to do that for individual neurons. 02:18:19.000 |
That's the first piece, and then the second piece is that it occurs to me 02:18:24.000 |
that there's so much rich understanding of the different states of sleep. 02:18:31.000 |

I feel like neuroscience is tasked, the field of neuroscience now is tasked 02:19:17.000 |
with giving us an understanding of the verb states 02:19:20.000 |
and like these waking states of mind are very mysterious, 02:19:25.000 |
because people wonder like is my focus poor or is it good? 02:19:32.000 |
We only tend to look at are they functional enough to do their job 02:19:38.000 |
We don't really have metrics, but for sleep we have metrics 02:19:41.000 |
and commercial products can measure that, sleep tracker rings, wristbands, 02:19:47.000 |
Well, yeah, I think modularity can actually be that metric. 02:19:52.000 |
Some metric of your large-scale organization of your brain can be that metric. 02:19:58.000 |
There's a number of labs that have done this, have measured modularity in real time, 02:20:02.000 |
so what I was talking about was just getting a snapshot 02:20:07.000 |
but we can also look at modularity has changes on a second-to-second-to-minute-to-minute basis, 02:20:12.000 |
and one of my former postdocs, Sepeda Sargiani, 02:20:16.000 |
she just did a very simple experiment where there were sounds 02:20:21.000 |
and the functional MRI scanner is very loud, so you can't hear very well, 02:20:26.000 |
but every once in a while there would be a sound that was just above the level 02:20:30.000 |
of the noise of the scanner, and all you had to do was sort of press a button 02:20:35.000 |
if you heard that sound, and you didn't pick it up all the time. 02:20:39.000 |
Maybe 80% of the time you heard it, and sometimes 20% of the time you didn't hear it. 02:20:44.000 |
Well, she measured their modularity on a moment-to-moment basis, 02:20:48.000 |
and she could predict if they were going to be correct or not 02:20:52.000 |

Can we extract this maybe out of heart rate variability or for oxygen? 02:21:43.000 |
A former student, Brian Miller, and a post-doc of Adam Ghazali's Westcliff, 02:21:49.000 |
who have a company called Neuroscouting, where they are able to-- 02:21:55.000 |
we've been sort of doing scanning and also collecting physiological data 02:22:02.000 |
we can measure the modularity in the scanner, 02:22:04.000 |

because they can collect oxygen and heart rate variability 02:22:11.000 |

Well, the sleep trackers of various kinds have certainly been able to pull out information 02:22:38.000 |
about rapid eye movement and other stages of sleep. 02:22:41.000 |
I mean, key metrics, not every metric, not what you would get with a person wearing an EEG probe 02:22:48.000 |
or something, a set of probes, but certainly information that can be used. 02:22:53.000 |
One thing that has me a little bit perplexed-- 02:22:56.000 |
and I'm almost reluctant to bring it up, but I'm going to do it-- 02:23:00.000 |
is that I did a couple episodes about psilocybin 02:23:04.000 |
and the use of psilocybin for the treatment of depression. 02:23:09.000 |

Psilocybin seems to induce fairly significant increases 02:23:58.000 |
in cross-modal talk between brain networks, in other words, less modularity. 02:24:07.000 |
No, it has to do with how we make these measurements, 02:24:17.000 |
And so I like to--when I think about connectivity, 02:24:20.000 |
we talked about this connectivity of brain state versus a brain trait. 02:24:26.000 |
So when we're talking about you being highly modular as a trait, 02:24:30.000 |
that's very different than what your modularity is like in different states. 02:24:34.000 |
It actually turns out when you do these highly executive demanding tasks, 02:24:40.000 |
you get less modular because your networks are communicating with each other. 02:24:46.000 |
So it's important for networks to get less modular when it's a more demanding task. 02:24:52.000 |
But that's very different than what's your baseline modularity, 02:24:58.000 |
because you've got to get from where your baseline is to this other state. 02:25:03.000 |
And a lot of it has to do with going from one state to another, 02:25:09.000 |
So that's interesting. I didn't know about those results, 02:25:11.000 |
but it's interesting that it does affect connectivity in that way. 02:25:16.000 |
I think the drugs that are going to be helpful 02:25:18.000 |
are going to promote networks talking to each other 02:25:23.000 |
as opposed to networks not communicating with each other. 02:25:26.000 |
In your clinic, do you ever combine drug therapies, cognitive training, 02:25:30.000 |

Yeah, so I think I have a lot of patients that I've referred for its approved use, 02:25:42.000 |

But we haven't really had any improved anything that's been for cognition. 02:25:54.000 |
There are a bunch of studies, anecdotal small studies, 02:25:57.000 |
where you can give transcranial magnetic stimulation, frontal cortex, 02:26:02.000 |
But they really haven't been done in ways that we don't know if it generalizes, 02:26:06.000 |
if it's going to be the way it's been done in depression in a way that can really be. 02:26:13.000 |
I think it will be part of the things we do, drugs, TMS, 02:26:24.000 |
What this is doing is really changing how nodes, you know, 02:26:30.000 |
It's not about sort of just increasing or decreasing activity 02:26:35.000 |
It's just sort of restoring the balance of a network. 02:26:42.000 |
